§ 8-5-2 Contents of application.
This section is included in your selections.
An application shall contain information reasonably required by the City Clerk, including, but not limited to the following:
(A) The name of the applicant;
(B) The address of the facility;
(C) The name, address and phone number of the manager of the facility;
(D) A brief description of the anticipated business activities;
(E) Whether a liquor license is to be applied for, and, if so, what class of license;
(F) A statement of the training to be required of all employees involved in the proposed pari mutual betting and/or any other authorized gaming activities. (Ord. 2001-754, passed 5-14-2001)
