(A) Introduction describing the reasons for the modification request as well as a general description identifying the parcels affected and the assessment values to be modified;

(B) Description of the proposed modification’s effect on the ratio of fair market value of each assessment parcel to the amount of the proposed assessment for each parcel affected;

(C) Description of the assessment methodology utilized in spreading the proposed assessments; for example, front footage, acreage, per lot or other methodology;

(D) A Status of Ownership in Title Report showing all parties having any interest in the affected parcels;

(E) Amended assessment diagram;

(F) A site map or vicinity map showing the location of the affected parcels in relation to other areas of the City for an area of approximately two to four miles, showing major streets;

(G) An electronic database (32 inch disk) reflecting parcel information after the modification (including, for each parcel, legal description, situs address, assessment amount and ownership name and address). (Prior Code, § 19-2-2) (Ord. 95-517, passed 1-10-1995)
